The Prison Boys is a puzzle game similar to novel puzzles set in the late Taisho era from 1912 to 1926 in Japan. The main character of the game is a spirit named Prison Boy. Tetsu Akatsuki breaks through 12 floors of the building to find out the truth about the prison with his partner Nam Saeki. The game's system for creating a main character is Def Con 1, in which Tetsu Akatsuki joins TOKKOH, a special police force under the command of Justice.

TOKKOH lives in the basement of a 12-story house. He owns the ground floor of the building, which houses The Prison Boys. Chung is TOKKOH's assistant and is hired to catch troublemakers and force confessions out of them. Tetsu starts to have doubts about Japanese nationalism and home when he begins his quest. He wears brown hair that's parted centerally and exhibiting red irises. He's a lower class citizen than the others in TOKKOH. Tetsu is more serious than the other characters. He believes in justice and has noticed TOOKOH's questionable morals as the game continues.

With Nagi Seaki, Tetsu's best friend and roommate, finding clues serves as a cooperative endeavor. He's somewhat reserved but very open about his feelings and can't stop the river from flowing. Naoya hides the fact that he loves his older brother from both parents. Nagi is also disliked by the family for his lack of consideration and deceptive behavior. You learn later in Prison Boys that Nagi will lead you to many different characters with unique personalities.

The Prison Boys's game play consists of chapters and stages. Each stage requires a ticket except for the discontinued stages the player exits before completing them. Complete the game by searching for the truth in 12 house risings. In Tetsu's initial dream, he sees a woman lying on the ground being killed by a man with a bloody sword. Nag and Tetsu come to suspect that Naoya, Nagi's brother, may have passed away. They investigate information regarding Naoya's death and discover many hidden truths about the organization. Both wake up from this nightmare with the same recurring dream for months. The game features a simple yet effective battle system and appealing graphics. Its age rating is consistent with overall appraisal of the game. A cooperative effort between two players reveals more about the game's secrets than any one can on their own.
